Report an incident

The guided incident report for a collision, an injury, a theft or anything else on the road.

Updated
5 October 2026

Do this after a collision, a near miss, an injury, a robbery, a theft, a police stop or any other incident with the car.

Before you start. Make sure everyone is safe. If anyone is hurt, call an ambulance on 913. Police is 911 and fire is 912. If you feel in danger, use SOS first. See SOS and incidents.

Steps

  1. Open the report: Report an incident on the ride screen, or Incident report under the Account tab or on the Safety screen.
  2. To photograph first, tap Take photos first. The camera opens at once and the questions come after.
  3. Pick what happened. The four most common kinds come first; More lists the others.
  4. Answer the questions in order: whether anyone is hurt, when and where, the photos, the other party and their vehicle, people hurt, witnesses, the police, and the damage to the car. Tap Skip for what you do not know.
  5. Write what happened in your own words.
  6. Tick True to the best of my knowledge. It is not an admission of liability.
  7. Tap Send report. If you cannot finish now, tap Send what I have now and finish within 24 hours.

What happens next

For a serious incident, such as a pedestrian or rider hurt, a robbery, or fire or smoke, operations is alerted as soon as the report reaches GuyWay. You do not need to finish first.

GuyWay tells its insurer. Once sent, the report cannot be changed, but you can still add a note, more photos and the police report. Operations may ask you questions in the app; they show as "Question from GuyWay operations" under Your reports.

When the police must be told, the report says so: report it at a police station or to an officer within 24 hours, and take the car's insurance certificate. This applies when someone is hurt, after a robbery or a hit and run, and whenever the other party left or did not give their details.
